Sleep deprivation does not disturb interventionist performance
A single-center study found that percutaneous coronary intervention (PCI) procedures performed during the middle of the night do not adversely affect the safety and effectiveness of procedures performed the next day by the same operator. Findings now available in Catheterization and Cardiovascular Interventions, a journal published by Wiley-Blackwell on behalf of The Society for Cardiovascular Angiography and Interventions (SCAI), show late night work while on call does not worsen performance of the interventionist doing PCIs the next day.
Studies have shown that sleep deprivation in medical personnel may increase the risk of serious medical errors, percutaneous injuries and motor vehicle accidents. Moreover, previous research has reported higher complication and failure rates for PCIs performed after regular working hours. However, to date no study has provided evidence of operator fatigue on the success and complication rates of PCI procedures. Each year more than one million PCI procedures, commonly known as angioplasty, are performed in the U.S. according to the National Heart, Lung, and Blood Institute.
For the present study, Vitalie Crudu, MD, and colleagues from Geisinger Medical Center in Danville, Pa., identified PCIs performed at the center from January 1, 2005 through December 31, 2009 between 11 p.m. and 7 a.m. PCIs performed during the following work day by the same interventionist were compared to PCI's performed by interventionists not on call the night before.
The team reported 3,944 PCIs were performed by four operators during the five-year period, which included 167 PCIs performed by operators after late night work while on call. The overall incidence of complications were similar in the PCIs done by operators working the night before compared with PCIs done by rested physicians. Types of complications identified as potentially more sensitive to operator error or judgement occurred with similar frequency in both groups. The post-call physician PCIs had 1.2% intra-procedural deaths compared to 0.2% in the rested physician group, a difference that was not statistically significant. Researchers reported more frequent excessive bleeding at the access site in the rested versus post-call PCIs at 2.7% and 0%, respectively.
"Our investigation found no evidence that middle-of-the-night PCIs adversely affect safety or efficacy of procedures performed the subsequent day by the same interventionist," concludes Dr. Crudu. "Further study with larger a larger number of cases is necessary to identify any warning signals of correlations between sleep deprivation and PCI outcomes." In the interim, the authors suggest that sleep-deprived interventionists could minimize risk by deferring emergency cases to rested colleagues and avoiding elective high risk PCIs while fatigued.

ScienceDaily (Mar. 29, 2012) ? How do you build a brain? In the March 30 issue of Science a team of investigators presents a surprising answer, reporting their discovery of a remarkably simple organizational structure in the brains of humans and other primates. Employing sophisticated mathematical analysis of advanced imaging data, they found that the pathways carrying neural signals through the brain are arranged not in a disorganized tangle but in a curved, three-dimensional grid.
"We found the brain is built from parallel and perpendicular fibers that cross each other in an orderly fashion. Finding this kind of simple organization in the forebrain of higher animals was completely unsuspected," says Van Wedeen, MD, of the Martinos Center for Biomedical Imaging at Massachusetts General Hospital, who led the study. "Knowing there is a simple plan that, modified by evolution and development, gives rise to all brains has implications for researchers working to build an atlas of brain connections, for pursuing investigation of how the brain develops and for expanding theories of how the brain works."
It is well known that neural pathways in the spinal cord and brain stem are organized in three principal directions -- head to tail, side to side, and front to back -- all of which are either parallel or perpendicular to each other and reflect the basic patterns of embryonic development. But following those pathways into the brains of higher animals -- particularly into the cerebral cortex -- has been challenging because each pathway crosses many others within a small space. Previous studies that followed the movement of tracer chemicals injected into specific brain regions could track the progress of single pathways only and could not be used in human participants.
In the current study, Wedeen and his colleagues used diffusion spectrum MR imaging -- a technology he developed that reveals the orientation of all fibers that cross a particular point on a pathway -- coupled with mathematical analysis of all crossing or adjacent pathways in the brains of four species of non-human primates and in human volunteers. Wedeen's previous studies of animal brains had revealed parallel crossing pathways that appeared to form sheet-like structures, but it was not clear whether those structures were pervasive or only characterized a few brain systems.
The analysis revealed that all crossing or adjacent fibers were either perpendicular or parallel to the original pathway. Each of the crossing fibers was, in turn, crossed by its own perpendicular fibers, interwoven like the threads in a sheet of fabric, that defined box-like, three-dimensional curved grid structures. The same grid-like pattern was seen throughout the white matter of the brains of all four studied non-human primates -- rhesus monkeys, owl monkeys, marmosets and galagos -- and the human volunteers.
"I don't think anyone suspected the brain would have this sort of pervasive geometric pattern," Wedeen says. "Although our findings could be described as a new longitude and latitude for the brain, they're also leading us to an entirely new understanding of how and why the brain is organized the way it is. The old image of the brain as a tangle of thousands of discrete, unconnected wires didn't make sense from an evolutionary standpoint. How could natural selection guide each of those wires into more efficient, advantageous configurations?
"The very simplicity of this grid structure is the reason why it can accomodate the random, gradual changes of evolution," he continues. "It's easier for a simple structure to change and adapt, whether we're talking about the big changes that occur across evolution or the changes that can occur during an individual's lifetime -- both the normal neuroplasticity associated with development and learning or the damage that results from injury or disease. A simple grid structure makes both evolutionary and develomental sense." Wedeen is an associate professor of Radiology at Harvard Medical School and on the faculty of the Harvard-MIT Health Sciences and Technology Program.

VANCOUVER, British Columbia (AP) ? Beach shots depict her as every inch a curvaceous beauty queen.
But 23-year-old Jenna Talackova was born male, and that led the Miss Universe Canada organizers to disqualify her last week as a finalist in the 61st Miss Universe Canada pageant taking place in May.
The rules of the contest run by the Donald Trump organization say entrants must be "naturally born" females.
The Vancouver woman underwent a sex change four years ago.
"She did not meet the requirements to compete despite having stated otherwise on her entry form," said a statement from Miss Universe Canada. "We do, however, respect her goals, determination and wish her the best."
The pageant's New York-based parent backed the decision.
"After review, organizers discovered that Jenna Talackova falsified her application and did not meet the necessary requirements to compete in the 2012 Miss Universe Canada pageant," a statement said.
The disqualification has won Talackova widespread sympathy and raised the question of whether the pageant has the right to decide who is female.
Talackova is obtaining legal counsel and is expected to make a statement next week, said her spokesman, Rory Richards.
Her change of gender was hardly a secret before the event because she had competed in the 2010 Tiffany Miss International Queen Competition for transgendered and transsexual women in Pattaya, Thailand. In a video interview for that pageant, she said she had lived her life as a female since age 4, began hormone therapy at 14 and changed sex at 19.
"I regard myself as a woman with a history," she said.
Connie McNaughton, Miss World Canada in 1984 and first runner-up for the world crown, called the decision outdated and discriminatory.
Some countries have their candidates undergo cosmetic surgery, she said, so what's wrong with sex-change surgery "because in your heart and soul you believe yourself to be a woman?"
A Vancouver transgendered activist, Jamie Lee Hamilton, said Talackova could sue for violation of her human rights.
"She was born with male genitalia and is being treated as a second-class citizen," Hamilton said. "Under the eyes of the law and the medical profession, she's a legal female."
Prof. Patrizia Gentile of Ottawa's Carleton University, who did a dissertation on beauty pageants, equated the ban with the exclusion of blacks and Jews from pageants in earlier times. "We're seeing more and more transgendered women wanting to be beauty contestants," she said. "The rule is incongruent with the culture."
Besides, she said, "Your genitalia have nothing to do with how you perform femininity."
Prof. Kathleen Lahey, a gender issues expert at Queen's University in Kingston, Ontario, said the organizers are looking at the issue wrongly; "If the organizers were being honest, they would recognize that this particular competitor was perhaps one of their most brilliant competitors ever ? to make it into the finals having performed the female gender so very well."
Spencer Chandra Herbert is among members of the British Columbia provincial legislature who are pushing legislation to guarantee equal rights in the area of gender identity. He said Talackova's exclusion shows how misunderstood the subject is.
"She may have had male bits when she was born but she's a female," he said. "I think Donald Trump and his Miss Universe contest need to get into the current century and get in line with the current science and respect that."
Trump's office said he was not commenting on the matter.
From the conservative side of Canadian society, Gwen Landolt, national vice president of REAL Women of Canada, said the pageant was simply being realistic in barring Talackova.
"Even though she had surgery and hormones, her DNA is still that of a man, his reproductive system is still that of a man," Landolt said.
"To consider yourself Miss Universe is to actually ask for more pain, suffering and rejection," Landolt said. "I'm sorry this person has put himself (or) herself though this."

During the course, delegates learn a range of health and safety legislation, aswell as developing risk management skills, and manual handling. Another popular option employers can choose is to have a safety officer visit the workplace to assess and train individuals and teach them how to follow guidelines correctly.
Health and safety at work does not have to be a pressure for employers as this burden can be relieved simply with either of these options.
For the most effective results, training should be repeated every few months, not annually. This allows for the training of new employees who might not possess the competence they require. It is a bad idea to have new staff working for you for long periods with no training. This method also regularly refreshes staff knowledge and skills.
Never compromise or cut corners when it comes to health and safety training for staff. Damaged property, loss of profit due to illness, accidents and delays are a few of the effects caused by untrained staff. Prevention is always better than cure.
You can also compliment content of the training using various relevant signs and posters. Signage and posters are a straightforward method to refresh the training content through effective reminders.
Effective health and safety training is essential for the success of a business. Everyone deserves to work in a secure environment that is free of hazards.

Forum for completely Out of Character (OOC) discussion, based around whatever is happening In Character (IC). Not entirely new to the Role Playing scene as I have been doing it for nearly 14 years and I am looking to start my own structured half medieval and modern with some fantasy elements on here. However I will be needing help to manage the game so here is what I seek for staff.
1. DMs to run quests within the room. Every week on Wednesday and Saturday quests will be held.
2. Anyone who is good at doing tallies by forum posts. The forum will be created however I just haven't figured out which site I plan to setup with yet. Although I am open for ideas and the forum must be free.
3. Moderators for the forum once it is setup.
4. Anyone who is experienced in doing HTML coding for webpages. The website I plan to create will have detailed Races that will be played within the game, Level and Experience Chart, Character Sheets, Detailed powers for each of the races as well as skills and abilities for the classes within the game. Shops where various Weapons, Armor, Rings, Amulets Etc that can be purchased with in game money and a Graveyard for characters who have died during quests or challenged to a death match by another character.

With ambition to become a big national company and possibly?go public soon, the company has expanded beyond solar to brand itself as the provider of a broader set of energy-related services, including upgrading air conditioning systems and insulating air ducts to minimize leaks to achieve savings.
The company is launching a marketing push for its energy efficiency service by announcing Tuesday a loan program from Admirals Bank of Boston. Homeowners can use the loan program to help finance energy efficiency audits and subsequent home improvement work, the cost of which typically ranges from $4,000 to $12,000, said Lyndon Rive, CEO of SolarCity in San Mateo, Calif.
?We are getting into more than electricity sales [to include] the full management of your energy at home ? everything that consumes energy in your house,? Rive said.
The loan, and the accompanying announcement that it?s expanding the energy efficiency service to the East Coast, provides a snap shot of a company that is a solid fund raiser and has helped to popularize the use of leases and power purchase agreements to remove the expensive barrier for going solar.
But competition has grown quickly in solar. The company recently?bagged an $81 million?equity round for expansion, technology development and acquisitions. An IPO would give it more capital to grow the different services it offers beyond solar, such as installing electric car charging stations.
Expansion mode
The company, founded in 2006, is making a push for its new services at a time when the business of installing solar systems has moved beyond the startup stage to attract a growing number of companies that have established brands. Vivint, known for its home security business,?lined up $75 million?last year to push its solar service. NRG Energy is now targeting the same kind of rooftop projects that SolarCity does after?buying Solar Power Partners?last year to create an in-house development team.
SolarCity, which has been going after larger and?larger projects, also is now competing more so than ever against companies such as SunEdison and ?SunPower which, incidentally,?filed a lawsuit last month?alleging data theft by former employees who went to work for SolarCity.
Then there are the roofing materials companies and electricians and roofers and various other businesses that have jumped into the solar market in recent years. Finally, not to mention venture-backed solar startups that have broadened their reach beyond their home state of California, such as Sungevity and Sunrun.
Growing the energy efficiency business is a key part to show SolarCity can scale beyond solar. The company hasn?t touted its energy efficiency services as much as it has its solar installation work, at least not in press releases, though Rive did invite Forbes into his home last year?for a video tour?to explain the results of an energy audit by his own staff.
Software and rebates
The company has worked on about 5,000 energy efficiency projects, some of them already completed,?since mid-2010?when it bought Building Solutions and launched the service in several western states and Texas. The company improved Building Solutions? software to do more accurate energy audits, recommend and prioritize upgrades and better quantify energy savings, said Levi Blankenship, manager of energy efficiency services at SolarCity.
While SolarCity offers leases for solar, it doesn?t provide similar financing arrangements for energy efficiency upgrades. Owning and operating solar energy systems and collecting monthly payments from their electricity production makes a good business case, but the same can?t be said for owning, say, furnaces, air duct insulation or water heaters, Rive said. So customers pay cash upfront for the retrofits, and some could turn to banks for home improvement loans. Federal, state and local incentives for energy efficiency upgrades also have helped to defray the costs for many.
And unlike the solar lease, which guarantees the amount of electricity produced each month, an energy efficiency contract from SolarCity doesn?t provide energy savings guarantees. It?s difficult to promise a certain amount of energy savings when the company can?t control how much energy a household uses, and that use can change with, for example, an addition of family members, Blankenship said.
The company does do a post-installation evaluation within a week or so to verify what energy savings will likely be if energy use doesn?t change, Blankenship said. This evaluation is sometimes necessary for claiming government rebates. SolarCity entered the energy efficiency business around the time when California started an incentive program called Energy Upgrade California?that gives rebates based on projected energy savings of a retrofit project rather than the cost of buying individual equipment. The company is now expanding its energy efficiency service to the East Coast regions, such as New Jersey, New York and Massachusetts, where it already offers solar installation.

Read a really interesting article today in the?Washington Post?called?Career Coach: Talk to employees before they leave.
The article spends a fair amount of time providing rationale about why exit interviews are something that should always be done but their value is suspect for a variety of reasons including:
Posing the right questions, or not.
Ill equipped interviewers.
Fear of burning bridges by the exiting employee.
Fear of a confidentiality breach; where does the info go and how is it used.
So they advocate the idea of doing exit-like interviews with employees while they are still at the business. They call them?Stay Interviews?and the whole idea is improve retention and overall employee performance by asking key employee satisfaction questions way before someone is thinking of leaving or actually leaves. By then it?s too late to react.
The concept of small business owners having one-on-one sessions with employees is hardly new. But translating those sessions into something structured that is specifically intended to improve retention is another matter. It doesn?t have to be complex, just planned and well executed.
Here?s how to implement such a plan:
1. Commit to a Process:?Lots of smallbiz CEOs hate the word process because they equate it to bureaucracy. But I do suggest telling people that you?re going to start this two way communication and put some structure/expectations around it so folks know that you?re serious.
2. CEO conducts 1-2 sessions per month:?Having the CEO do the feedback sessions is key. The feedback loop starts and stops right where it should, at the top. Frequency: Anything less than 1-2 times per month and you might as well not do it at all. Anything more frequent and they risk becoming rushed and discounted by all involved.
3. Interviews should be casual but consistent:?This means that the CEO should have an established list of questions to ask or topics to discuss but the session should be conducted in a non-threatening, conversational manner.
4. Confidentiality:?At some point, the CEO will share the aggregated information with the company. But individual employee feedback should only be shared with the consent of the employee. Importantly, leakage of any information gleaned in the sessions could completely undermine or even destroy the program objectives.
5. Communicate expectations:?It?s super important that the CEO NOT attempt to address every single employee concern or suggestion. The idea is to aggregate feedback into common themes and address them holistically?where appropriate.
6. But do communicate:?I recommend that at the monthly company wide meetings, a consistent item on the agenda is the feedback that the CEO has garnered from his/her ?stay? sessions. Side note: If ?a smallbiz CEO is NOT conducting monthly company meetings, then there are much more fundamental issues to deal with at that company.
7. Address what make sense, but not everything:?Lastly, the CEO and management need to carefully consider all suggestion and feedback and not necessarily react to every one. Some ideas will be great, others not so great and a bunch in between. For consistency and transparency, consideration of all ideas needs to be evaluated relative to company mission and objectives.?Note: If a smallbiz doesn?t have a mission and objectives that are shared with employees, then there are much more fundamental issues to deal with at that company.
This suggested approach is not complicated and needn?t take up a lot of time for a CEO. Some initial thought and planning is key and then its all about executing on a consistent regular basis. This whole thing can badly backfire without consistent implementation and communication.

This download in particular includes Sense 3.6, but throws in third-party Beats Audio support -- a feature that until now has only been seen in version 4.0. If you're packing a Vivid, just make sure you're hooked up to WiFi and head to the settings to get your download on. If it doesn't find the update yet, you also have the option to sideload it from your computer.
In the same breath, AT&T also made it known that it's still hard at work readying ICS for several other devices, and we can expect the updates to arrive "in the coming months." There aren't any surprises on the list of candidates: the Motorola Atrix 4G and Atrix 2, Pantech Burst and Element and the LG Nitro are all there, as well as a litany of Samsung devices like the Note, Galaxy S II, Skyrocket, Captivate Glide and Galaxy Tab 8.9. YOKOHAMA, Japan (AP) ? Nissan's revival of the Datsun won't include developed markets, Chief Executive Carlos Ghosn said Wednesday, outlining a strategy to target the revamped brand at emerging nations where the biggest growth is expected for affordable stylish vehicles.
The offerings will be tailored for each market, including price and engine size, targeting the burgeoning market of first-time car buyers in countries such as India, Indonesia and Russia, where Datsun will be introduced from 2014, he said.
Executives gave away few details, including specifics of Datsun models they had in the works.
Corporate Vice President Vincent Cobee said the new Datsuns will be entry-level cars in each nation, aimed at "up and coming" successful people who are "optimistic about the future."
He said two models will go on sale within the first year in the three nations, and an enhanced lineup of models will be offered within three years.
Nissan Motor Co. faces intense competition from rivals, including other Japanese players such as Toyota Motor Corp. and Honda Motor Co., which all have their eyes on emerging markets that also include China, Mexico and Brazil. Growth has stagnated in recent years in more established markets such as Japan, the U.S. and Europe.
Ghosn announced Tuesday in Indonesia that the Datsun will make a comeback, three decades after the shelving of a brand that helped define not only Nissan but Japanese carmaking in the U.S., as well as in Japan. It is a name that is synonymous with affordable and reliable small cars, according to Nissan.
Datsun debuted in Japan in 1932, and hit American showrooms more than 50 years ago.
It was discontinued globally starting in 1981 to unify the model lineup under the Nissan brand. Nissan also makes Infiniti luxury models.
Tsuyoshi Mochimaru, auto analyst with Mitsubishi UFJ Morgan Stanley Securities, said the Datsun name helps differentiate cheaper models targeting emerging markets from Nissan's other models.
"Emerging markets are where the growth is, but what will sell are cheaper cars, where profit margins will be lower," he said. "By separating the brand, you avoid hurting the value of the Nissan brand."
The new blue Datsun logo was inspired by the old one, according to Nissan.
Ghosn said Nissan has been preparing the Datsun brand for years and was already developing models. He was confident Nissan had not fallen behind rivals.
"We just see an opportunity," he told reporters. "When you're ready, you should do it."
Nissan officials acknowledged only the older generation may be familiar with the Datsun brand and it may not even be recognized in emerging markets.
But they said it stood for good styling, durability and safety.
"Datsun is part of the heritage of the company," Ghosn said. "Datsun is a good name."
